Welcome to Midwest Orthopaedic Center. We look forward to providing you with comprehensive, compassionate orthopedic care. Here are some tips to help you prepare for your first visit with us:

Request an appointment. Call our office Monday through Friday, between 8:30 a.m. and 4:45 p.m. Our centralized scheduling number is (309) 691-1400. From Pekin, please call (309) 353-4159.

Bring your insurance card, any co-payment and photo ID. Please check with your insurance to verify coverage and any co-payment that is due for specialty care. Also please find out if you need a referral from your primary care doctor for specialty care. Treatment under Workers’ Compensation or any liability insurance must be pre-authorized in writing by your employer or Workman’s Compensation carrier.

Arrive early. Please arrive at least 30 minutes before your first appointment to allow time to complete the registration process, including scanning insurance card(s) and completing any forms needed, as well as a health history with the clinic staff.

Complete your medical history form. For your convenience, you can download the form here. Please print and fill out the form associated with your provider and bring it with you to your appointment.

Bring a list of current medications. Please bring a list of all medications you are taking, including over-the-counter medications, vitamins and supplements.

Have you had X-rays or other imaging tests? If you had your diagnostic exam (X-ray, MRI, CT, sonogram) at any Peoria or Pekin hospital or their clinics, Midwest Orthopaedic Center has computer access to your results. If your exam was done elsewhere, please bring a CD of the images and the interpretation report.

Appointments for Minors: Midwest Orthopaedic Center requires that a parent or legal guardian accompany any minor (under the age of 18) to the first appointment. This allows for a proper discussion of treatment options and informed consent. If for subsequent visits a parent or legal guardian cannot accompany the minor, a Permission to Treat a Minor without a Parent or Guardian Present form must be completed, signed, witnessed and sent with the person accompanying the minor to the appointment. All co-payments are due at the time of service, so the accompanying adult should be prepared to make any payments due. You can download the form here.
