From your first appointment to your last follow-up, we tailor your treatment to your needs, with a care plan and timeline that work for you.

That’s what makes us your home for orthopaedics. We strive to keep you comfortable with an easy check-in, candid consultation and straightforward access to some of the most advanced orthopaedic tools and technologies available in the Midwest.

It all starts here –– at your first visit.

First, request an appointment that fits your schedule. Just call our office during the week, Monday through Friday, between 8:30 a.m. and 4:45 p.m.

This will allow you an appropriate amount of time to complete our new-patient registration process, provide a valid identification card and insurance card, and fill out whatever forms are needed.

Before coming to your appointment, confirm that your insurance provider will cover your visit. You’ll want to verify whether you need a referral from your primary care physician, as well.

Remember, for workers’ compensation and liability insurance coverage, we require a written pre-authorization from your employer or workers’ compensation carrier.

The standard medical history form can be found here. Please print it and fill it out, then bring it to your appointment.

To give your doctor a complete picture of your medical health, bring a list of your current medications –– including over-the-counter medicine, vitamins and supplements.

We have general access to any X-rays, CT scans, MRIs, sonograms or other imaging tests that you may have had completed at local hospitals and clinics. If your imaging was done outside of Peoria or Pekin, please bring a CD of the images as well as any associated reports.

Midwest Orthopaedic Center requires a parent or legal guardian to accompany any minor (child under the age of 18) to their first appointment. We have this rule in place so that doctors can discuss treatment options with informed consent.

When a parent or legal guardian cannot be at a subsequent appointment, a Permission to Treat a Minor without a Parent or Guardian Present form must be completed, signed and witnessed. This form is to be provided at check-in by the adult accompanying the minor.

The accompanying adult must also be prepared with a form of payment, since all payments are due at the time of the appointment.
