Treatment under workers’ compensation must be pre-authorized in writing by your employer or workers’ compensation carrier. No appointment will be made if you do not have a claim number, prior approval and remittance information for the claim. If your claim is denied, you may elect to use your medical insurance, but you are responsible for payment of all co-payments and deductibles. We will need a written denial from your Workers’ Compensation carrier or employer.

If a Workers’ Compensation carrier denies further authorization for patients already under care by Midwest Orthopaedic Center, it is the patient's responsibility to contact their health insurance carrier to receive approval for continued treatment.
