If you got hurt on the job, we make it our mission to get you back to work as soon as possible. At Midwest Orthopaedic Center, we know how to treat workers’ compensation injuries, returning homebound employees to their jobs with a care plan that works for them.

If your jobsite injury is covered under workers’ comp, you’ll need to bring the following to your first appointment –– as provided by your employer or workers’ compensation carrier:

  • Claim number
  • Nurse case manager name and phone number
  • Adjuster name and phone number
  • Valid photo ID

If your claim is denied, it is your responsibility to provide any copays or deductible payments as well as a written denial from your employer or workers’ compensation provider.

In the event your workers’ comp coverage is denied, please contact your health insurance carrier to confirm whether they’ll cover existing charges and future treatment.
