Making an Appointment

Appointments may be scheduled or changed by calling (309) 691-1400 Monday through Friday between 8:30 a.m. and 4:45 p.m.  If you are unable to keep a scheduled appointment, please contact us so that we may offer the time to another patient.  We recognize that your time is valuable, and we will make every effort to avoid unnecessary waits.  However, delays may occur when patients with emergencies are added to our regular schedule.  We appreciate your understanding and patience in this matter.

Depending on your insurance, you may or may not be able to self-refer. Please call your insurance to verify coverage and to determine if you need a referral from your general physician. Please keep in mind some MOC physicians require a referral even if not stated by your insurance carrier.
